Responsibilities
  • Coordinate and manage patient care plans throughout the healthcare continuum
  • Perform comprehensive patient assessments and identify discharge planning needs
  • Collaborate with physicians, nurses, social workers, therapists, and other healthcare professionals
  • Facilitate appropriate utilization of healthcare resources and services
  • Develop and implement individualized care plans to support optimal patient outcomes
  • Coordinate referrals, home health services, rehabilitation services, and community resources as needed
  • Advocate for patients and families while ensuring continuity of care
  • Monitor patient progress and adjust care plans accordingly
  • Maintain accurate documentation and comply with regulatory and facility requirements
  • Participate in interdisciplinary care conferences and case reviews
Requirements
  • Active RN License in Missouri
  • Minimum 2 years of recent Case Management, Utilization Review, Discharge Planning, or Acute Care nursing experience
  • Current BLS Certification required
  • Certified Case Manager (CCM) certification preferred
  • Strong knowledge of care coordination, discharge planning, and healthcare regulations
  • Excellent communication, organizational, and problem-solving skills
  • Ability to manage multiple cases effectively in a fast-paced environment
  • Commitment to delivering high-quality, patient-centered care
