- Contract Length: 13 Weeks
- Shift: Day Shift
- Schedule: 5x8
- Experience Required: 2 Years
- Weekly Pay: $2100 to $2300
Responsibilities
- Coordinate and manage patient care plans throughout the healthcare continuum
- Perform comprehensive patient assessments and identify discharge planning needs
- Collaborate with physicians, nurses, social workers, therapists, and other healthcare professionals
- Facilitate appropriate utilization of healthcare resources and services
- Develop and implement individualized care plans to support optimal patient outcomes
- Coordinate referrals, home health services, rehabilitation services, and community resources as needed
- Advocate for patients and families while ensuring continuity of care
- Monitor patient progress and adjust care plans accordingly
- Maintain accurate documentation and comply with regulatory and facility requirements
- Participate in interdisciplinary care conferences and case reviews
Requirements
- Active RN License in Missouri
- Minimum 2 years of recent Case Management, Utilization Review, Discharge Planning, or Acute Care nursing experience
- Current BLS Certification required
- Certified Case Manager (CCM) certification preferred
- Strong knowledge of care coordination, discharge planning, and healthcare regulations
- Excellent communication, organizational, and problem-solving skills
- Ability to manage multiple cases effectively in a fast-paced environment
- Commitment to delivering high-quality, patient-centered care
