Roles & Responsibilities
- Coordinate patient care plans across multidisciplinary teams
- Advocate for patients and families throughout their healthcare journey
- Ensure smooth transitions between care settings
- Collaborate with physicians and specialists to optimize outcomes
- Document and track patient progress with attention to detail
Qualifications
- Active RN license in Florida or compact state
- Case Management experience preferred
- Strong communication and organizational skills
- Ability to work independently and as part of a team
Knowledge & Skills
- Patient advocacy and care coordination
- Discharge planning expertise
- Electronic medical records proficiency
- Knowledge of healthcare regulations and compliance
