In lieu of BSN, RN must have 5 years case management experience and complete a Bachelor of Science in Nursing within 2 years of hire date. 5 years of nursing experience with 2 years critical care or 3 years nursing experience with 2 years case management experience preferred. InterQual experience preferred. Ability to establish and maintain collaborative and effective working relationships. Ability to communicate effectively in oral, written, and electronic formats. Demonstrates analytical and critical thinking abilities with proactive decision-making and negotiation skills. Description: The RN Case Manager is responsible for promoting patient-centered care by coordinating the plan of care for the patient's stay. They manage the length of stay, ensure appropriate resource management, and develop a safe discharge plan in collaboration with the multidisciplinary team. The RN Case Manager facilitates the progression and transition of care using established criteria. They coordinate activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. The RN Case Manager provides case management services for both inpatient and observation patients as assigned. They identify patients who are at risk for adverse outcomes during the transition from one level of care/setting to another. The RN Case Manager performs comprehensive assessments of psychosocial, medical, and discharge needs of patients and families. They reassess the patient's clinical condition as indicated and consider the patient's readmission status or risk of readmission. The RN Case Manager develops strategies to mitigate these risks, including education on accessing healthcare resources and preventative education. They coordinate the plan of care and drive the discharge plan by collaborating with the multidisciplinary healthcare team. The RN Case Manager is responsible for timely referral to Social Services when risk factors for psychosocial determinants of health are identified. They involve the patient, family, and significant others in identifying and clarifying needs to develop mutual and realistic goals. The RN Case Manager evaluates the progression of care using evidence-based tools throughout the episode of care. They escalate progression and transition of care issues through the established chain of command. The RN Case Manager makes appropriate referrals to third-party payer and disease management programs for recurring patients. They facilitate patient throughput with a focus on effective care transitions, quality, and efficiency. The RN Case Manager documents recommendations, discharge plans, care coordination interventions, and activities to communicate effectively with all members of the healthcare team. They align patient needs with available resources to ensure a safe discharge or transition. The RN Case Manager acts as a liaison through effective communications with physicians, patients, families, hospital staff, and outside agencies. They actively seek ways to control costs without compromising patient safety, quality of care, or the services delivered. The RN Case Manager directs activities to identify and provide for the needs of the under-resourced patient population, including education and community-based resources. They participate in performance improvement activities by identifying and documenting avoidable days. The RN Case Manager adheres to established policies, procedures, and standards of care and escalates issues through the established chain of command.
