Days | Monday–Friday | 8:30 AM – 5:00 PM | 40 Hours/Week
- Coordinate discharge planning and case management for medical and emergency department patients.
- Collaborate with physicians, social workers, and the interdisciplinary care team to ensure safe, timely patient transitions.
- Assess patient needs, coordinate post-acute services, and facilitate appropriate utilization of healthcare resources.
- Manage a caseload of approximately 20 patients while maintaining accurate documentation and communication.
