We are seeking a dedicated and experienced Registered Nurse (RN) to serve as a Case Manager within our healthcare organization. - Facility: HCA Florida Bayonet Point Hospital
- Travel Pay: $2206/week
Duties:
- Conduct thorough patient assessments, including vital signs, medical history, and physiological evaluations, to determine care needs and develop individualized care plans.
- Coordinate inpatient and outpatient services, facilitating communication among physicians, specialists, therapists, and other healthcare providers.
- Manage discharge planning processes by evaluating post-acute care requirements and arranging appropriate services to ensure smooth patient transitions.
