Registered Nurse - Case Management
Location: Fort Pierce, Florida
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, rotating every other weekend
Hours per Week: 40
Candidate Pay Rate: $2000- $2150 Weekly

Roles & Responsibilities
- Coordinate seamless care transitions as a vital RN Case Manager
- Collaborate within a dynamic RN/SW/CMA triad model to support patients
- Perform utilization review using Interqual criteria for initial and continued stay reviews
- Develop comprehensive discharge plans that empower patients and families
- Adapt quickly to high-acuity units and float across the facility as needed
- Leverage tools like Meditech, Midas, and Outlook to streamline case management workflows
Qualifications
- Active RN license in Florida or Compact state
- 2-4 years of RN Case Management experience in an acute care setting
- Comfortable working in a fast-paced, high-acuity Level II trauma environment
- Previous HCA experience is a plus, along with familiarity using Meditech, Midas, and Outlook
Knowledge & Skills
- Strong case management and discharge planning expertise
- Proficiency with utilization review processes and Interqual guidelines
- Ability to float across multiple units and adapt to changing priorities
- Excellent communication and collaboration within multidisciplinary teams
