Description:
Typical census ranges from 180 to 230 patients.
The unit has 6 total case managers.
One case manager is available for a weekend option, Saturday and Sunday from 7 AM to 7 PM.
There are 4-5 case managers scheduled Monday to Friday from 8 AM to 4:30 PM.
Patient ratios are 1 case manager to 40 patients Monday to Friday.
Staff utilizes laptops and cell phones.
The electronic medical record system used is MediTech and Midas.
Candidates must have case management experience and familiarity with Midas and InterQual.
Knowledge of area resources is preferred but not mandatory.
Typical duties include interviewing patients and families face-to-face.
Daily multidisciplinary rounds are conducted on each unit.
Every patient undergoes InterQual reviews and observation reviews.
Discharge planning is a critical component of the position.
Ideal candidates will be adaptable and be able to get along well with others.
The position may require floating to various departments within the organization.
Scrub color is Caribbean blue, and a white jacket is part of the attire.
The unit adheres to established guidelines and policies.
Rotating weekends may be required, specifically Saturday and Sunday from 8 AM to 4:30 PM.
No on-call duties are required.
