Conduct comprehensive assessments of client needs, including medical, social, and financial factors. Develop, implement, and update individualized care or service plans. Coordinate services across providers and community resources. Monitor client progress and adjust plans as needed. Advocate for clients to ensure access to appropriate services and resources. Maintain accurate and timely documentation in compliance with organizational and regulatory standards. Educate clients and families on available resources and care plans. Facilitate communication between clients, families, and healthcare or service providers. Ensure compliance with policies, procedures, and applicable regulations. Participate in interdisciplinary team meetings and case reviews.
