Job Title: RN - Case Management Profession: RN Specialty: Case Management Duration: 13 Weeks Shift: Day Shift Hours per Shift: 8:30 AM - 5:00 PM Experience: Minimum of 1 year of acute hospital case management experience License: Valid RN License Certifications: N/A Must-Have: Acute hospital case management experience Description: This is a contract position for an RN in Case Management. The assignment is for a duration of 13 weeks. The weekly schedule is from 8:30 AM to 5:00 PM. Experience in acute care is required. No on-call duties are required, though weekend shifts may be necessary. The unit consists of 34 beds with a staff composition including 1 day shift RN and 1 social worker for case management. Candidates must be collaborative, organized, and able to multitask. Effective and efficient communication skills are essential. Dress code includes navy blue scrubs and business casual attire for non-scrub responsibilities. Candidates will perform tasks such as discharge planning, chart review, and patient screening. This role involves care coordination and barrier mitigation tasks. Preference will be given to candidates with over 2 years of acute hospital case management experience.
