Locations will vary, and specific duties will be defined after candidate selection. The position will cover multiple areas, and nurses need to be located in these regions. The contract nurse will serve as a floater, stepping in when regular nursing staff are on leave. Key functions will include conducting nursing assessments and collaborating with external agencies. Assessments will authorize skilled nursing services for members. The successful candidate will conduct in-person visits statewide and monitor health needs. They will assist with referrals and advocate for member rights. The role involves completing reports to document medical necessity for skilled services. In-person assessments will be required every 90 days as needed. The position is field-based, and travel is necessary for assessments. Duties include completing nursing referrals, placing vendor calls, and participating in multidisciplinary planning meetings. Collaboration with members and their support systems is essential. Attendance at mandatory meetings and completion of required annual trainings is expected.
