Job Title: RN Care Manager / Case Manager

Contract: Travel Assignment

 

Job Description:
Seeking an experienced RN Care Manager with at least 2 years of acute care Case Management experience focused on Care Coordination and Discharge Planning. The RN will collaborate with multidisciplinary teams to coordinate patient care, facilitate safe discharges, and ensure appropriate utilization of healthcare resources.

Requirements:

  • Active California RN License (pending license accepted but must be active by start date)
  • Minimum 2 years of acute care Care Management experience
  • Experience with Care Coordination and Discharge Planning required
  • First-time travelers accepted (screening required)

Assignment Details:

  • Start Date: ASAP
  • Floating: None
  • Patient Ratio: 1:25 (Days)
  • Weekend Requirement: Typically one full weekend every 4 weeks (based on department needs)
