Responsibilities:

Perform comprehensive assessments of high-risk members to develop tailored care plans.
Collaborate with primary care providers to ensure effective care delivery.
Coordinate care transitions and liaise with physicians, social workers, and discharge planners.
Document clinical, functional, and fiscal outcomes of patient management.
Act as a patient advocate, ensuring privacy, confidentiality, and adherence to treatment plans.
Identify opportunities for health promotion and illness prevention.
Monitor for adverse patient outcomes and intervene promptly to minimize poor outcomes.
Support non-medical needs such as housing and transportation to ensure compliance.
Maintain detailed records and ensure documentation includes skills competencies, elder abuse, HIPAA, infection control, and OSHA training.
