Delivers patient care by incorporating principles of Relationship Centered Care and through the application of the nursing process based on theory, research, and evidence-based practice. Assesses and evaluates patient care needs and applies critical thinking skills in patient care management. Integrates relevant assessment and intervention skills in the delivery of nursing care. Collaborates with the interdisciplinary healthcare team to develop, implement, and evaluate the plan of care. Prioritizes all aspects of patient care, including teaching, rounding, coaching, and planning after hospital care. Educates patients and caregivers about their plan of care, transition of care, health promotion, and disease prevention. Communicates effectively and professionally with patients, families, and all members of the healthcare team. Manages assignments within the Care Delivery Model of Modified Primary Nursing. Demonstrates patient-focused and cost-effective approaches to patient care in terms of equipment, staff, supplies, and other resources. Documents nursing practice accurately and thoroughly in compliance with regulatory and legal standards. Engages in professional development to support the environment of care for clinical colleagues and patients. Incorporates ethical principles into decision-making for patients and their families. Encourages lifelong learning and innovation for self and others. Participates in shared decision-making through nursing department committees and unit initiatives. Practices safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement. Demonstrates clinical skills and knowledge crucial to quality and safety in the patient population served. Implements quality improvement strategies based on evidence and institutional standards. Collaborates with teams to create and implement quality improvement projects and nursing research studies. Acts to establish and maintain a therapeutic relationship with patients and families while addressing their needs and preferences. Initiates service recovery for patients and families who express concerns. Supports staff use of evidence-based practices to enhance patients' perceptions of care. Conducts Purposeful Hourly Rounding on assigned patients addressing their needs. Escalates potential safety hazards or gaps in best practices as needed. Facilitates patient throughput on shift basis.
