Travel Medical Social Worker in Fairfield, CA

Position: Medical Social Worker – Hospital $$SN$$
Location: Fairfield, California
Shift: Days – 5x8s (8:00 AM – 4:30 PM)
Duration: 8 Weeks

• Provide direct clinical social work services, counseling, discharge planning, and case management to hospitalized patients and their families.
• Conduct comprehensive psychosocial assessments to identify emotional, social, financial, environmental, and discharge planning needs.
• Develop and implement individualized psychosocial plans of care in collaboration with patients, families, physicians, nurses, and interdisciplinary care teams.
• Facilitate discharge planning and coordinate referrals to community resources and support services.
• Monitor patient progress toward goals of care and identify barriers affecting length of stay or discharge readiness.
• Provide crisis intervention, grief counseling, advocacy, and support for patients and families coping with illness, loss, and life transitions.
• Document assessments, interventions, and care plans accurately within the medical record and designated electronic systems.
• Participate in quality improvement initiatives, care coordination activities, and interdisciplinary team meetings.
• Support continuity of care across the healthcare continuum and promote optimal patient outcomes.
• Perform additional duties as assigned.
