Travel Medical Social Worker - $39 to $50 per hour in San Ysidro, CA

• Position: Master Social Worker (MSW)
• Location: San Ysidro
• Contract Length: 26 Weeks
• Shift: Day Shift
• Schedule: 5×8 (Monday–Friday)
• Experience Required: 1 Year
• Pay Rate:$29 to $50 per hour
• Language Requirement:Bilingual – English & Spanish (Required)

• Conduct comprehensive psychosocial assessments for patients and families
• Develop individualized care plans that address emotional, behavioral, financial, and social needs
• Provide crisis intervention, counseling, and emotional support to patients and caregivers
• Connect patients with community resources, social services, housing assistance, transportation, and financial support programs
• Coordinate care with physicians, nurses, behavioral health providers, case managers, and other interdisciplinary team members
• Assist patients with discharge planning and continuity of care
• Advocate for patients to ensure access to appropriate healthcare and community resources
• Educate patients and families regarding available support services and treatment options
• Maintain accurate documentation in the electronic medical record (EMR)
• Ensure compliance with organizational policies, ethical standards, HIPAA regulations, and state and federal guidelines
• Promote culturally sensitive, patient-centered care while maintaining confidentiality

• Master's Degree in Social Work (MSW) from an accredited institution
• Active California MSW license or registration, as required by the facility
• Minimum 1 year of recent social work experience in a healthcare, outpatient clinic, community health, or behavioral health setting
• Fluent in both English and Spanish (Required)
• Current BLS Certification preferred
• Strong knowledge of community resources, case management, and psychosocial assessment
• Excellent communication, counseling, crisis intervention, and documentation skills
• Experience using electronic medical record (EMR) systems preferred
• Ability to work independently while collaborating effectively with multidisciplinary teams
