Job Title: Medical Social Worker Profession: Social Services Specialty: MSW (Masters of Social Work) Duration: 13 Weeks Shift: Day Hours per Shift: 8 Hours (08:00 - 16:30) Experience: Minimum of 1 year of acute care experience required License: CA state license required; Pending license accepted but must be licensed upon start date Certifications: LCSW, BLS, MSW Must-Have: - Caseload of 20-23 patients - Every other weekend availability - 1 year of acute care experience Description: Responsible for conducting initial patient evaluations. Develop plan of care to meet patient and caregiver needs. Establish and revise goals based on patient assessments. Review health history to facilitate accurate care planning. Assist patients in navigating social services processes. Coordinate home care and equipment services following discharge. Document essential information to support care coordination. Participate in care conference activities and interdisciplinary team meetings. Provide crisis intervention as needed. Educate patients and caregivers to facilitate safe discharge. Refer patients to rehab and long-term care facilities as required. Collaborate with internal and external team members for quality patient care. Maintain documentation according to care plans and ensure compliance with care standards. Support patients with obtaining assistance programs such as Medicaid and Medicare. Conduct needs assessments to identify necessary services and support. Manage patient discharge planning and address post-acute care needs. Work with a variety of patient ages and diagnoses. Foster strong communication with medical teams and case managers. Prepare patients for transitions of care to the lowest level needed. Maintain knowledge of cultural issues and ethical standards in social work. Assist with high-risk interventions and develop safety plans as required. Engage with patients experiencing behavioral health challenges in a supportive manner. Demonstrate a commitment to high-quality, patient-centered care.
