Job Title: Home Health LMSW Profession: Social Services Specialty: LMSW (Licensed Masters of Social Worker) Duration: 13 weeks Shift: Day Hours per Shift: 5x8-Hour (08:00 - 16:30) Experience: 1 year of Skilled Home Health Experience required License: Must have both OR and WA licensure (WA license can be in the application process) Certifications: BCLS Must-Have: Oasis experience required Description: Home Health LMSW position available in East Portland, OR and surrounding areas. Traveler must possess both OR and WA licensure. The start date is as soon as possible. Shifts are 8-hour days. This facility does not accommodate 4x10s or 3x12s. The patient ratio is 1:1, with the possibility of visiting up to 6 patients per day. A productivity points standard of 30 is expected. First-timers accepted. No weekend requirements. No on-call requirement. Travelers are not required to float between units. Block scheduling is not accommodated. Locals accepted within a 50-75 mile radius at the same rate. The facility may call off up to one shift per pay period. Travelers must have their own transportation. The system effectively groups clients to minimize drive and windshield time, with occasional maximum travel of roughly 60 miles total per day. Primary series COVID vaccination is required, in addition to one booster. Business casual attire is expected, with no specific scrub color required. Charting is done using EPIC Remote Client for Home Health and Rover for mobile tasks. Mileage reimbursement is provided. References must cover 1 year within the last 3 years, including charge RNs or leads. Travelers are expected to float between facilities within a given state. Refusal to float may result in termination of assignment. Travelers required to use their phones for work-related tasks will be reimbursed up to a certain limit. Modules for training are required and will not be billable. Average time spent on modules is expected to be around 16 hours.
