Job Overview
We are seeking a compassionate and dedicated Social Worker to join a supportive healthcare team in West Valley, UT. This role is perfect for someone who can balance empathy, advocacy, and problem-solving while helping patients navigate life’s challenges one step at a time.
You will work closely with patients, families, and interdisciplinary teams to provide emotional support, discharge planning, community resource coordination, and crisis intervention services.
Responsibilities
- Assess patients’ psychosocial needs and develop appropriate care plans
- Provide counseling, emotional support, and crisis intervention services
- Coordinate discharge planning and connect patients with community resources
- Collaborate with physicians, nurses, case managers, and healthcare staff
- Advocate for patient rights and ensure continuity of care
- Maintain accurate and timely patient documentation
- Assist patients and families with coping strategies and support services
- Ensure compliance with healthcare regulations and facility policies
Requirements
- Minimum 2 years of Social Worker experience required
- Master’s Degree in Social Work (MSW) preferred
- Current state Social Worker license/certification as required
- BLS certification preferred
- Strong communication, organizational, and interpersonal skills
- Ability to manage sensitive situations with professionalism and compassion
