- Weekly Pay: $1800 to $2000
Responsibilities
- Provide emotional support and counseling to patients and families
- Assess patient social, emotional, and financial needs
- Develop discharge plans and coordinate community resources
- Advocate for patients to ensure access to appropriate healthcare services
- Collaborate with physicians, nurses, and interdisciplinary healthcare teams
- Assist patients with crisis intervention and care planning
- Maintain accurate and timely case documentation
- Educate patients and families regarding available support services and resources
Requirements
- Active Social Worker license or certification as required by Arizona
- Minimum 2 years of healthcare or medical social work experience
- BLS Certification preferred
- Strong communication, counseling, and problem-solving skills
- Ability to manage sensitive situations with professionalism and empathy
- Excellent organizational and case management abilities
