Job Summary
Provide occupational therapy services to patients in their homes by evaluating functional abilities, developing individualized treatment plans, and promoting independence through therapeutic interventions. Utilize OASIS documentation while collaborating with an interdisciplinary home health team.
Responsibilities
Perform comprehensive occupational therapy evaluations in the home setting.
Develop and implement individualized treatment plans.
Complete OASIS documentation accurately and timely.
Educate patients and caregivers on adaptive techniques, equipment, and home safety.
Collaborate with physicians and the interdisciplinary home health team.
Maintain accurate clinical documentation and treatment records.
Travel throughout the assigned coverage area to provide patient care.
Required Qualifications
Minimum 2 years of current Occupational Therapy experience
Minimum 2 years of current Home Health experience
OASIS experience
Active Michigan Occupational Therapy License
Current AHA BLS Certification
Reliable personal vehicle with a clean driving record
Proof of auto insurance
Preferred Qualifications
Strong independent clinical decision-making skills
Previous rural home health experience
Excellent time management and scheduling abilities

Mileage reimbursed at the current federal rate (calculated from the main office)

All scheduling requests and RTO must be submitted with the initial profile or they will not be considered
