Job Details:

  • Contract Duration: 13 Weeks

Travel Pay for 40 hrs Evenings.

  • $1,700 Weekly Gross
  • W2: $16/Hr.
  • Stipend: $1060 Weekly

Requirements: 

  • NY State LPN/LVN license - Required
  • Minimum 1 year of recent experience in LTC
  • EPIC Experience - Required
  • Current BLS AHA certifications.

Job Summary:
We are seeking a dedicated Licensed Practical Nurse (LPN) specializing in LTC to join our healthcare team. The Licensed Practical Nurse (LPN) works under the supervision of the RN to provide direct patient care for patients medically ready for discharge but experiencing delays due to various barriers. This role involves working collaboratively with a multidisciplinary team, and assigning/supervising tasks to LPNs, Certified Nursing Assistants (CNAs), and unregulated assistive persons (UAPs). The LPN maintains an in-depth knowledge of nursing principles, practices, standards and techniques.
Duties

  • Perform thorough patient assessments, including vital signs, physical examinations, and physiological evaluations to inform care plans.
  • Utilize EMR (Electronic Medical Record) systems to document patient information meticulously and ensure compliance with HIPAA regulations.
  • Assist with invasive procedures such as IV insertions, catheterizations, and sterile dressing changes using aseptic techniques.
  • Monitor patients’ conditions continuously, including ventilator management and acute pain management strategies.
