Job Description – Travel Registered Nurse (RN) | Long-Term Care (LTC)  

Location: Claremont, NH

Duration:13 weeks

Key Responsibilities

• Monitor residents' physical and mental health, identify changes in condition, and communicate findings to the Nursing Supervisor and physicians.
• Receive, transcribe, and implement physician orders accurately.
• Administer medications, IV therapy (as applicable), treatments, oxygen therapy, enteral feedings, wound care, and wound VAC management according to physician orders and facility policies.
• Maintain accurate electronic documentation, including MARs, TARs, resident assessments, and nursing notes.
• Supervise Licensed Nursing Assistants (LNAs), delegate assignments appropriately, and ensure quality resident care in accordance with New Hampshire Board of Nursing regulations.
• Participate in physician rounds and interdisciplinary care planning.
• Coordinate admissions, discharges, and resident transfers while completing all required documentation.
• Monitor infection prevention practices, maintain a safe unit environment, and ensure regulatory compliance.
• Complete daily reports, review new physician orders, monitor residents receiving antibiotics or isolation precautions, and communicate resident concerns to nursing leadership.
• Communicate effectively with residents, families, physicians, and the interdisciplinary team.
• Manage assigned nursing unit during the shift under the direction of the Nursing Supervisor or Director of Nursing.
• Maintain controlled substance accountability, including narcotic counts at shift change.
• Assist with orientation, education, and mentoring of new nursing staff as assigned.

Required Qualifications

• Active New Hampshire Registered Nurse (RN) License (must be active and in hand at the time of submission).
• BLS Certification (American Heart Association).
• Minimum 3 years of current RN experience within the last 6 months.
• Willingness to float to units where clinically competent.
• IV Certification or willingness to obtain IV certification during onboarding.
• COVID-19 vaccination required (medical/religious declinations are not accepted).
• Candidates must live more than 50 miles from the facility.
• Ability to work in a rural healthcare environment with reliable personal transportation
