Description
LanceSoft is seeking a LPN / LVN Continuous Care Coordinator for a travel job in Grand Rapids, Michigan.
Job Description & Requirements

Licensed Practical Nurse (LPN) – Continuing Care
Location: Grand Rapids, MI
Contract Length: 13 Weeks (or per facility need)
Shift: 12x3 Nights
Hours: 7:00 PM – 7:30 AM (12-Hour Shifts)
Weekend Requirement: Every Other Weekend
Responsibilities
- Provide direct patient care under the supervision of an RN, Physician, or Mid-Level Provider.
- Collect and monitor patient assessment data and report changes in condition.
- Administer treatments and medications as directed.
- Educate patients and families regarding treatments, medications, and care plans.
- Document patient care accurately in the electronic medical record.
- Collaborate with the interdisciplinary healthcare team to ensure continuity of care.
- Maintain a clean and safe patient environment.
- Participate in quality improvement and patient safety initiatives.
- Uphold patient privacy, confidentiality, and ethical standards.
- Perform other duties as assigned.
Requirements
- Graduate of an approved Practical Nursing program.
- Active Michigan LPN License.
- BLS Certification (AHA or ARC) required within 90 days of hire.
- Successful completion of competency-based orientation.
- Previous healthcare experience preferred.
