Job Requirements

  • Education Requirements: Valid Licensed Vocational Nurse (LVN) license from the state of Texas.
  • Certifications Requirements: Current CPR and First Aid certifications; Ventilator/Tracheostomy certification (preferred); Medication Administration certification.
  • Experience Requirements: Minimum of 2 years of LVN experience in long-term care, hospital, or group home settings; Proficiency with g-tubes/enteral feedings, Foley catheters, and tracheostomies; Experience caring for medically fragile residents with behavioral and mental health challenges.
  • Other Requirements: Ability to work evening shifts (2p-10p); strong clinical skills; excellent communication and teamwork abilities; adaptability to shift schedule changes; physical ability to assist residents with mobility and daily activities.

Job Responsibilities

  • Provide high-quality nursing care to residents with complex medical needs, including those with behavioral challenges and mental health issues.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each resident’s condition and needs.
  • Maintain accurate and timely documentation of resident assessments, treatments, and progress reports within the EMR system.
  • Monitor residents’ vital signs and patient responses to treatments, especially during enteral feedings, Foley catheter management, and tracheostomy care.
  • Execute emergency procedures confidently when residents experience acute health episodes, ensuring resident safety and compliance with facility protocols.
