Job Title: Clinical Social Worker Profession: Behavioral Health Specialty: Clinical Social Worker Duration: Full Time Shift: Days Hours per Shift: Guaranteed Hours Experience: Minimum Years of Experience License: Licensed Clinical Social Worker (LCSW), Professional Counselor (LPC), or Marriage and Family Therapist (LMFT) preferred. Certifications: Basic Life Support (BLS) within 3 months. Must-Have: Master’s Degree in Social Work, Psychology, Counseling, or a related discipline from an accredited educational institution. Valid Driver’s License or ability to obtain within 6 months of hire. Description: This position provides evidence-based culturally relevant outpatient behavioral health treatment for individuals experiencing behavioral, emotional, and mental disorders. Clinical services include individual, group, family, and marital therapy. Services also encompass bio-psychosocial assessments, diagnostic evaluation, care coordination, and crisis intervention. The clinician is an engaged member of the multidisciplinary treatment team supporting individuals and families. The clinician maintains electronic health records with timely documentation of services in accordance with established policies and procedures. The clinician follows and adheres to established clinical productivity standards. This position involves providing after-hours crisis intervention services on a shared rotating basis. Clinicians who are not yet licensed will receive clinical supervision that meets licensure requirements. Supervision will focus on the appropriate delivery of core functions including screening, intake, assessment, treatment planning, therapy, case management, crisis intervention, education, referrals, and consultation. Key Essential Functions and Accountabilities: Implement culturally relevant screening, assessment, diagnosing, treatment planning, and evidence-based individual, group, and family therapy to meet clients' needs. Utilize Trauma Informed Care in all interactions. Integrate mental health and substance use disorder interventions based on individualized assessments and treatment planning. Develop individualized treatment plans and reviews based on individual needs. Provide care coordination, advocacy, motivational interviewing, bio-psychosocial education, and skill development. Maximize access to care by improving coping strategies, promoting self-care, and managing symptoms. Communicate and collaborate with clinical and paraprofessional team members to achieve treatment goals. Provide crisis intervention services during normal clinic hours. Maintain complete, accurate documentation of all services within the electronic health record as required by policy and procedure. Self-monitor and fulfill licensure requirements as needed. Adhere to patient confidentiality protocols according to regulations. Coordinate care within the health system and community social agencies to ensure proper referrals. Engage in staff meetings and multidisciplinary team meetings. Meet productivity expectations to support access to timely care. Respond respectfully and professionally to requests and corrections. Complete all required enrollment expectations and documentation. Additional duties may be assigned as needed. Experience and Knowledge Required: Experience with behavioral health assessment and diagnoses. Ability to provide assessments, treatment plans, and referrals for patients of all ages. Knowledge of behavioral health assessment and diagnostic evaluation criteria. Expertise in treatment plan development and patient-centered goals. Familiarity with evidence-based methods used in therapy for emotional and mental health issues. Understanding of federal and state mandatory reporting requirements. Awareness of ethical and legal practices and documentation regulations. Skills and Abilities: Assess patients for risk of self-harm and develop effective safety plans. Advocate for patients and coordinate required services in crisis situations. Analyze and diagnose situations, adopting effective courses of action. Document clear reports based on patient observation, screenings, and assessments. Modify treatment activities as needed based on changes in patient needs. Stay informed on new developments in behavioral health assessment and counseling.
