Responsibilities
  • Conduct comprehensive psychosocial assessments of patients and families
  • Develop individualized care plans and coordinate appropriate support services
  • Provide counseling, emotional support, and crisis intervention services
  • Assist patients and families with discharge planning and care transitions
  • Connect patients with community resources, financial assistance programs, and support services
  • Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams
  • Advocate for patients' rights and ensure access to appropriate care and resources
  • Educate patients and families regarding available services, treatment plans, and community programs
  • Maintain accurate and timely documentation in accordance with facility policies and regulatory standards
  • Ensure compliance with patient confidentiality, safety, and quality care guidelines
Requirements
  • Master's Degree in Social Work (MSW) preferred
  • Active California Social Work license (LCSW or as required by the facility) preferred
  • Minimum 2 years of recent healthcare, hospital, hospice, or clinical social work experience
  • Experience with discharge planning, case management, and crisis intervention preferred
  • Strong knowledge of community resources and healthcare systems
  • Excellent communication, counseling, and interpersonal skills
  • Ability to work effectively in a multidisciplinary healthcare environment
  • Strong organizational and problem-solving abilities
  • Commitment to delivering compassionate, patient-centered care
