Job Title: Intake Therapist Profession: Social Worker Specialty: Adult Behavioral Health Duration: Full Time Shift: 8:30 AM - 7:00 PM Hours per Shift: 9.5 hours Experience: Minimum of one year post-Master’s experience providing mental health treatment services to adults with severe mental illness. License: Must possess the corresponding state licensure. Certifications: Candidates must obtain the Qualified Mental Health Professional and Qualified Intellectual and Developmental Disabilities Professional credential within 120 days of hire. Must-Have: Must possess a CAADC credential or be on an active, approved registered certification Development Plan within six weeks of hire. Description: We have an exciting opportunity for an Intake Therapist to join our team of talented professionals. Regular working hours are Monday through Thursday from 8:30 AM to 7:00 PM and Friday 8:30 AM to 5:00 PM. Some evening or weekend hours will be required. Services are delivered in the office setting on a walk-in or scheduled basis. Flexible part-time hours are available, and full-time positions may be considered. Conduct intake assessments to identify areas of need and service preferences and complete documentation within 48 hours. Gather and integrate information from available sources at the time of intake, including consumer self-reports and documented medical history. Utilize motivational interviewing techniques to assess the motivation of consumers to address specific needs identified during the assessment process. Provide complete and accurate information regarding services and supports available to consumers, family members, and guardians. Assist consumers with initiating the person-centered planning process consistent with principles of Person-Centered Planning and current Medicaid Guidelines. Develop Safety Plans as required based on needs identified during the assessment process. Identify consumers who require more extensive evaluation and facilitate referrals for additional resources as needed. Consider and promote the utilization of group and support services. Participate in regularly scheduled clinical supervision, case consultations, and team meetings. Document all service activity and communications regarding consumers. Meet acceptable standards of productivity and complete all documentation within expected timeframes. Respond in writing to routine chart reviews and audits. Maintain appropriate licensure or certification. Complete 25 hours of program-specific training annually. Uphold professional ethics and boundaries. Abide by all organizational policies and procedures. Other duties may be assigned. Mileage reimbursement is available when a personal vehicle is used for work-related travel.
