Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, occasional weekends every 3-4 weeks, flexibility for ED coverage 10:00 AM - 6:30 PM

Candidate Pay Rate: $50 per hour

Roles & Responsibilities
- Conduct comprehensive social work assessments to support patient needs
- Perform mental health assessments and Title 36 petitions as required
- Develop discharge plans that pave the way for smooth transitions
- Address homelessness, substance abuse, and SDOH through targeted consults
- Complete maternal child assessments and handle DCS/APS reporting
- Serve diverse areas including inpatient, ED, Family Birthing Center, and ICU
Qualifications
- Active LMSW or LCSW license
- At least 1 year of experience
- Available for profile review and manager interview
