Job Title: Home Health Licensed Masters of Social Worker Profession: Social Services Specialty: LMSW (Licensed Masters of Social Worker) Duration: 13 weeks Shift: Day Hours per Shift: 8 hours (08:00 - 16:30) Experience: 1 year of skilled home health experience required License: Must have licensure in both Oregon and Washington (Washington license can be in the application process) Certifications: BCLS required Must-Have: Oasis experience required Description: The position involves providing home health services in a designated area. Travel between patients is expected, with the possibility of visiting up to 6 patients per day. The work involves documentation and coordination with both internal and external team members. The job requires the traveler to have their own transportation, as travel distances may vary. The average maximum driving radius is approximately 60 miles per day. All travelers must comply with primary series COVID vaccination and one booster. Business casual attire is required, with scrubs allowed but not mandatory. Charting will be done using an EPIC remote client and a phone application. The facility promotes minimizing drive time between clients. Weekends are not required for this position. All travelers are expected to float between all facilities within the designated state. Refusal to float may result in termination of assignment. References covering recent experience are required. Candidates should possess skills in areas such as care coordination, discharge planning, and patient education.
