We are seeking an experienced Licensed Master Social Worker (LMSW) or RN Case Manager/Discharge Planner to join a fast-paced acute care hospital setting. The role focuses on discharge planning, care coordination, and ensuring safe and effective transitions of care for adult and geriatric patients.
