The Licensed Clinical Social Worker (LCSW) provides comprehensive psychosocial assessment, intervention, counseling, and discharge planning services for patients in an acute care hospital setting. This role collaborates closely with physicians, nursing staff, case managers, and interdisciplinary teams to address patients' emotional, social, and environmental needs.
The ideal candidate will have recent hospital-based social work experience and a strong understanding of behavioral health, crisis intervention, and care coordination.
Key Responsibilities
- Conduct comprehensive psychosocial assessments for patients and families.
- Develop individualized care plans and coordinate appropriate interventions.
- Provide crisis intervention, counseling, emotional support, and advocacy services.
- Collaborate with physicians, nurses, case managers, and community agencies to facilitate continuity of care.
- Coordinate discharge planning and referrals to community resources, behavioral health services, rehabilitation programs, and post-acute care facilities.
- Assist patients and families with adjustment to illness, hospitalization, and treatment plans.
- Identify barriers to care and develop solutions to support safe transitions.
- Maintain accurate and timely documentation in the electronic medical record.
- Participate in interdisciplinary rounds and patient care conferences.
- Ensure compliance with hospital policies, regulatory requirements, and professional standards.
