We are seeking an experienced Licensed Clinical Social Worker (LCSW) for a 13-week contract assignment at a hospital-based setting in Canton, GA. The ideal candidate will have a strong background in acute care and be skilled in providing comprehensive social work services to diverse patient populations.
This role involves collaborating with interdisciplinary teams to support patient care, discharge planning, and psychosocial assessments.
Responsibilities- Conduct psychosocial assessments and develop care plans
- Provide crisis intervention and emotional support to patients and families
- Collaborate with physicians, nurses, and care teams for discharge planning
- Coordinate referrals to community resources and support services
- Maintain accurate and timely documentation
