The role involves working collaboratively within a multi-disciplinary team. You will be responsible for conducting biopsychosocial assessments. The candidate will develop plans of care that meet patient and caregiver needs. Effective communication with patients and families is essential. You will coordinate services to ensure quality and cost-effective care. Experience in navigating the Department of Children and Families process is preferred. Crisis intervention skills are necessary for this role. The candidate should have knowledge of advance directives and patient rights. Understanding of various social issues and community resources is crucial. Ability to document care delivery and patient responses accurately is required. Experience in working with diverse patient populations is essential. This role may involve educating patients and families regarding support and resources. Flexibility to work with different age groups, including pediatrics and geriatrics, is expected. Participation in care conference activities and collaboration with external team members is required.
