Care coordination is a significant aspect of this position. The candidate will participate in case management activities. Experience with child protective services is necessary. The position involves collaborative care and addressing drug/alcohol use and treatment options. High-risk intervention for patient vulnerability or safety risks is required. The candidate must be familiar with the NASW Code of Ethics and Standards of Home Health Care. Participation in care conference activities is expected. The clinical focus is primarily on pediatrics. Referrals to other disciplines and resources are a part of the role. Experience in suicide, homicide, and self-harm interventions is required. Working with a multidisciplinary team to ensure continuity of care is essential.
