Job Title: Medical Social Worker Profession: Medical Social Worker Specialty: Hospital Duration: 8 weeks Shift: Days Hours per Shift: 8:30 AM - 5:00 PM, Mondays through Fridays Experience: Minimum years of experience required License: LCSW License required Certifications: None specified Must-Have: Strong psychosocial assessment skills Strong critical thinking skills Strong clinical decision-making skills Skilled at working with patients and families coping with loss and grief Excellent follow-through skills Strong organizational skills with attention to detail Description: Provides direct clinical social work services, counseling, discharge planning, and case management services to patients and their families. Conducts assessments and documentation of emotional, social, financial, environmental, or follow-up referral needs. Works collaboratively with the interdisciplinary healthcare team. Formulates a psychosocial plan of care based on assessment of patients' unique strengths and needs. Develops a discharge plan for the designated caseload. Monitors progress toward goals of care and identifies barriers to meeting estimated length of stay. Facilitates integration with community-based resources to optimize patient health. Activities are consistent with mission and values and in compliance with applicable regulations and policies and procedures. Partners with physicians, nursing, and other members of the healthcare team to establish, document, implement, and evaluate a plan of care based on patients' unique health strengths and needs. Brings psychological assessment skills and interdisciplinary team expertise to the team. Monitors progress of the plan of care and communicates any changes with the care team. Provides integrated mind, body, spirit service to the patient and family through a mutually agreed-upon plan of care and patient advocate relationship. Supports processes related to health management of patients across the continuum of care, including quality improvement and clinical research. Assesses needs of patients and families and conceptualizes problem situations within the social work scope of service using appropriate age-related skills. Provides direct social work services and counseling to patients and their families. Interprets social, psychological, emotional, financial, and family issues for attending healthcare team members. Completes documentation required in patients' medical records on department forms and reports. Demonstrates continued enhancement of professional skills. Complies with department and hospital policies and procedures. Supports organizational goals to provide optimal care to patients and families. Performs other duties as assigned. Must be willing and able to float to other departments as requested and qualified. Ability to work effectively with diverse populations, including those with respect to age, culture, and gender. Demonstrates respect for others' perspectives. Physical effort includes some standing, walking, stooping, bending, twisting, and periods of sitting while using a computer keyboard throughout the workday.
