Job Title: Licensed Clinical Social Worker Profession: Social Services Specialty: Case Management Duration: 13 Shift: Day 5x8-Hour Hours per Shift: 8 Experience: Minimum of 1 year experience required License: LCSW, MSW Certifications: BLS, LSWAIC Must-Have: - Inpatient/Acute Care experience - Case Management experience - Epic experience - Hospital experience Description: Perm candidates from any other facility are eligible for a travel contract if they have been separated for at least three months. One year of experience is required. First-time travelers are accepted. Travelers may be floated to other campuses if the census is low. Weekend coverage is required for a minimum of two weekend days per month. Typically, this means working one Saturday and one Sunday per month, but not in the same weekend. The facility specializes in care coordination for neuro and cardiac patients. It is also a comprehensive stroke center. Clinicians need to have prior hospital experience. References covering one year within the last three years are required during the compliance process. Modules averaging 16 hours spent may be necessary. Time spent on modules is self-reported, and completion will be documented. Candidates should have experience with various social services and assessments, including adult protective services, advance directives, and care coordination. Skills in discharge planning, crisis intervention, and knowledge of cultural issues are essential.
