Providing counseling for individuals, families, and groups; referring patients for psychiatric evaluation and treatment as needed. Assessing, advising, advocating, and assisting patients in obtaining benefits. Developing linkages between providers, social service agencies, and other resources to ensure continuity of care. Identifying and addressing barriers to care and retention in the program. Participating in case conferences with providers to ensure service coordination and continuity of care.
