Night Shift | 6:45 PM – 7:15 AM
We are seeking an experienced Licensed Clinical Social Worker (LCSW) for a travel assignment at a large acute-care hospital in North Carolina. This position offers the opportunity to work collaboratively with interdisciplinary healthcare teams while supporting patients and families throughout their care journey.
Position Details:
• 13-week travel contract
• Night shift: 6:45 PM – 7:15 AM
• Floating to various units as needed
• Documentation systems: Cerner and post-acute care platforms
Responsibilities:
• Collaborate closely with RN Case Managers and healthcare teams
• Assess patient psychosocial needs and develop appropriate care plans
• Provide counseling, support, and discharge planning services
• Coordinate resources and community services for patients and families
• Maintain accurate and timely documentation
• Build trusting relationships with patients, families, and care teams
• Prioritize and manage multiple responsibilities in a fast-paced environment
