Description:

Our client is currently seeking a Travel IHA Nurse Practitioner – Genesee County, NY

This is a per‑diem, field‑based role performing in‑home Initial Health Assessments (IHAs) for Medicare, Medicaid, and ACA members. Providers will travel within a 50‑minute radius, with the highest needs concentrated in the Rochester, NY region. This assignment requires PCP or home‑health experience, with a preference for candidates who have both.


This job will have the following responsibilities:

  • Perform in‑person annual preventative health risk assessments within members’ homes.
  • Provide a minimum of 15 appointment slots per month.
  • Complete all required self‑paced and instructor‑led training within designated timelines.
  • Ensure high‑quality assessments that meet member satisfaction and health plan expectations.
  • Document and submit chart notes within 24 hours of each visit.
  • Address documentation queries within 48 hours.
  • Maintain clear and proactive communication with clinical leadership.
  • Attend periodic training sessions and meetings.
  • Travel within a 50‑minute coverage radius (zip codes: 14585, 14450, 14475, 14453, 14472).
  • Safely enter and work within members’ personal living environments.
  • Lift up to 20 lbs and bend/kneel for extended periods if required.

Qualifications & Requirements:

Education and/or Certification Requirement

  • Master’s or Doctorate from an accredited NP program (FNP, AGNP, ANP)
    OR Master’s degree from an accredited PA program.
  • Current RN and NP or PA licensure in state of practice.
  • Board certification (ANCC, AANP, NCCPA).
  • Current Medicaid ID and Medicare PTAN.
  • Current BLS certification.
  • Valid driver’s license and adequate auto insurance.

Experience Requirement

  • Home health experience required within the last 5 years.
  • In‑home assessment experience highly preferred.
  • PCP experience strongly preferred (updated requirement).
  • Strong background in EMR documentation and patient communication.

Management Requirement

  • Ability to work independently while managing own daily visit schedule.
  • Self‑directed time management to meet documentation and turnaround expectations.

Process Oversight Requirement

  • Ensure all assessments meet quality, compliance, and regulatory standards.
  • Follow CMS and ACA risk‑adjustment models for accurate HRA documentation.
  • Maintain HIPAA compliance for patient information.

Specific Skills Requirement

  • Strong computer and EMR skills.
  • Excellent verbal/written communication.
  • Ability to make sound decisions within protocol.
  • Flexible with days, evenings, and weekend scheduling.

Additional Requirements

  • Providers must be credentialed with Excellus, NYS Medicare, and Medicaid.
  • Mileage reimbursed after the first 30 miles/day at current IRS rate.
  • Minimum 4 hours/visits per day for the first 8 weeks.
