Description
American Traveler is hiring an RN Case Manager for a Hospice/Palliative Care position in Loudoun County, VA, requiring a minimum of 1 year of recent hospice experience and a valid VA or compact RN license.
Details
- Community/home-based hospice setting operating out of a hub office in Loudoun County, VA
- Geriatric patient population requiring end-of-life care
- Caseload of approximately 5–7 patients per day
- Day shift schedule, 5x8-hour shifts (8:00 AM – 5:00 PM)
- No on-call responsibilities
- Territory covers up to a 60-mile radius from the hub office; flexibility with territory is expected
- EMR: Netsmart
- iPad provided for use in the field
Requirements
- Active VA RN license or compact RN license currently in hand
- Current BLS certification required
- Minimum 1 year of recent hospice nursing experience required
- Strong time management skills with an emphasis on timely documentation and communication
Additional Information
- Responsibilities include managing a caseload of hospice patients in a community/home-based setting across an assigned territory
- Scrubs required in any color; jeans are not permitted
- Mileage is reimbursed at the IRS standard rate
