Job Requirements
  • Education Requirements: High school diploma or equivalent; minimum of 2 years of hospice care experience
  • Certifications Requirements: BLS certification, Valid Driver’s License, Auto Insurance
  • Experience Requirements: 2 years of hospice or end-of-life care experience, proficiency with EMR systems (EPIC preferred), excellent communication skills
  • Other Requirements: Valid driver’s license and reliable vehicle, ability to explain hospice care effectively, comfort with end-of-life medication management, flexibility to travel within the Sheboygan/Manitowoc area and occasionally to Green Bay or Oshkosh, professional appearance, and strong interpersonal skills

Job Responsibilities
  • Provide compassionate end-of-life care to hospice patients in private homes, SNFs, and assisted living facilities within the designated territory.
  • Explain hospice services, expectations, and medication management to patients and families, ensuring they are well-informed and comfortable with care plans.
  • Collaborate closely with physicians, case managers, and liaisons to coordinate patient care and obtain necessary orders.
  • Complete two patient admissions per shift, or conduct visits if admissions are unavailable, maintaining detailed documentation in EPIC EMR.
  • Assess patient symptoms, manage pain and comfort measures, and educate providers on medication protocols and symptom management at end of life.
