**Join the Team as a Home Health RN!** **Job Description:** - Are you passionate about providing quality care in the comfort of patients' homes? - Do you thrive in a dynamic environment where every day brings new challenges? - If you are a skilled RN looking to make a tangible impact, we want you on our team! **Key Responsibilities:** - Deliver skilled nursing and coordinate comprehensive care to patients and families in their own residences. - Conduct thorough observations, assessments, and evaluations while planning effective interventions tailored to each patient’s unique needs. - Supervise care provided by Home Health/Hospice Aides and CNAs, ensuring adherence to the plan of care. - Foster continuity of care through effective communication with interdisciplinary teams, community health resources, and social agencies. - Execute quality care and symptom management strategies, promoting optimal health and enhancing patients' quality of life through education and support. - Maintain accurate and up-to-date patient records in compliance with Agency standards and OASIS requirements. - Participate in team scheduling coordination and an on-call rotation to meet patient needs effectively. - Engage in quality improvement initiatives and continuing education as part of your professional development. - Perform additional duties assigned by your supervisor to support our mission. **Minimum Qualifications:** - Current RN license in the State of Montana (BSN preferred). - Valid driver’s license with reliable transportation and appropriate auto insurance. - A minimum of 2 years of recent experience in acute care, home care, or hospice, with clinical skills in venipuncture, catheterization, wound care, and parenteral therapy preferred. **Schedule:** - Flexible schedule; typical in-home RN hours are M-F, 8-5, with on-call rotation. - Holiday coverage required. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
