Description

American Traveler is hiring a Home Health RN with 2 years of recent acute care, home health, or hospice experience for a 13-week assignment requiring an active MT or compact RN license.

Details

  • Provide skilled nursing care and case management to patients in home or in-patient Hospice Center settings
  • Coordinate and integrate services across an interdisciplinary team to ensure continuity of care
  • Oversee care provided by Home Health/Hospice Aides and CNAs to assigned patients
  • Typical schedule is Monday through Friday, 8am-5pm, with on-call rotation as needed
  • 40 guaranteed hours per week over a 13-week assignment

Requirements

  • Active MT or compact RN license required
  • Minimum 2 years of recent acute care, home health, or hospice experience preferred
  • BSN preferred
  • Valid driver's license, appropriate auto insurance, and reliable transportation required
  • Clinical skills such as venipuncture, catheterization, wound care, and/or parenteral therapy preferred

Additional Information

  • Admit patients onto services and negotiate, procure, and coordinate resources needed by patients and families
  • Assess and evaluate patients' physical and psychosocial status, including changes in condition and reactions to treatment
  • Provide patient and family education, counseling, and appropriate interventions to promote positive clinical outcomes
  • Maintain accurate and complete patient records in accordance with agency standards and OASIS requirements
  • Participate in on-call rotation, assessing patient needs by phone and making home visits as required
  • Participate in quality improvement activities and required in-service training
