Job Summary:
Home Healthcare Registered Nurse (RN) Simi Valley, California. This role involves planning, organizing, and directing home care services to meet patients' needs. The position serves as a resource for patients, families, and team members to ensure optimal care delivery.

Job Details:

  • Schedule: Day shift, Monday through Friday, 8:00 am to 5:00 pm, with a minimum of 40 hours per week. Rotating weekends required.
  • Required Qualifications: Bachelor's Degree in Nursing (BSN) preferred; acute care facility or home care experience preferred. Valid Registered Nurse (RN) licensure in the state of practice; Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS or HS-BLS or RQIBLS) certification from an approved vendor per healthcare policy. Valid Driver's License (DL) at least 21 years or older, with current automobile insurance and a reliable vehicle.
  • Essential Functions: Conducts initial and ongoing comprehensive assessments, documents all patient interactions, updates care plans, educates patients and families, and ensures compliance with legal and procedural standards. Performs on-call duties as assigned based on operational needs.
