**Join Our Team as a Physical Therapist in Fremont, CA!** - Are you a passionate and qualified Physical Therapist looking for a rewarding opportunity in a dynamic environment? - We invite you to join a dedicated team serving the vibrant areas of Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, Oakland, Berkeley, Alameda, Albany, Emeryville, and Piedmont. - Embrace the chance for additional travel outside your assigned area and the opportunity to work weekends if desired. **Key Highlights:** - **Productivity Requirements:** - For guaranteed hours, achieve at least 27 points per week. - For non-guaranteed hours, maintain flexibility in caseload with options to refuse patients. - **Visit Types and Points:** - OASIS ReCertification, OASIS RoC, Routine, Eval, Discharge, Eval SoC, Eval RoC, Eval ReCert, Eval F/up, Re-Eval, Eval Discharge, Oasis Discharge, Oasis F/up = 1 point = 1.33 hours. - OASIS SoC = 1.5 points = 2 hours. - **Travel and Coverage:** - Cover travel time, visit time, documentation time, and patient communication time within the all-inclusive rate. - Required to see Covid+ and Covid Pui patients. **Qualifications:** - Current CA PT License - BLS Certification - Valid Drivers License and Auto Insurance - Auto Registration - TB Test (annual) - MMR, Varicella, HepB, TDap (10 years), Flu (annual) - Drug Screen - Physical Examination **Experience:** - Minimum 1 year of experience required. **Why Catalytic Solutions (CatSol):** At CatSol, we connect dedicated therapists with rewarding assignments across the country.
