Job Summary:
The organization is seeking a Physical Therapist (PT) with Home Health (HH) specialization for a position located in Rancho Cordova, California. This role involves providing outpatient clinical services in home environments, focusing on a variety of diagnoses including orthopedic, cardiovascular, pulmonary, oncology, neurological, and geriatric conditions. The position offers an opportunity to work within a dedicated team serving a broad service territory, with a focus on patient care continuity and operational efficiency.
Job Details:
- Schedule: Days; Monday through Friday, 8:30 AM to 5:00 PM, with a minimum of 40 hours per week.
- Required Qualifications: Valid Physical Therapist license in California; current BLS (AHA) certification; minimum of 2 years of healthcare experience, including home health and OASIS experience; valid drivers license and proof of automobile insurance; strong oral and written communication skills; good organizational skills.
- Preferred Qualifications: EMR HomeCare Home Base (HCHB) experience; community or home health experience preferred.
- Additional Information: The role involves providing treatments in homes, assisted livings, senior apartments, and care homes, with approximately 3 to 5 patients daily. The caseload typically includes two Start of Care visits and evaluations, with an emphasis on work quality within the home environment. The position requires work on one weekend/month (not mandatory), holiday work, and adherence to a business professional dress code. Orientation will be three weeks, including online learning and field precepting, with solo work expected by the end of the second or third week. Mileage reimbursement is not provided.
