Description

American Traveler is seeking an experienced RN for a dual-role Home Health and Hospice position in the Rapid City, SD area, requiring a valid SD or Compact RN license and at least 1.5 years of home health experience.

Details

  • Dual-role position serving both Home Health and Hospice patients in a field-based setting
  • Three primary service areas covered: Rapid City (up to 80 miles outside the city), Custer/Hill City, and Hot Springs — specific area assignment will be discussed during the interview
  • Typical caseload is 5–6 routine patients per day plus approximately 2 admissions per day, with each RN managing a shared panel of 20–25 patients alongside an LPN, seeing each patient approximately once per week
  • Each RN is paired with an LPN, with designated Case Managers per region and a Clinical Resource Nurse available for support
  • CNA support is provided directly to patients in the home; nurse is responsible for lab draws, IV therapy, and vascular access in the home setting
  • Charting is performed in Epic Remote
  • Schedule is 8-hour day shifts, 40 hours per week
  • Weekend rotation required every third weekend for day shifts
  • One 12-hour on-call shift per week required (8:00 PM – 8:00 AM)
  • Holiday coverage is expected and will be discussed during the interview
  • No floating required
  • Free parking available

Requirements

  • Active RN license — South Dakota (SD) or Compact state license required
  • Current BLS certification required
  • Valid driver's license required; must be able to drive at night
  • Minimum 1.5 years of RN experience required
  • Home health and/or hospice experience required
  • Proficiency with OASIS documentation required
  • Clinical skills required include: wound care, wound VACs, PICC line dressing changes, port access, IV infusions, catheter insertion, drain management, and infection control practices
  • Familiarity with hospice philosophy required
  • Two professional references dated within the past year required for consideration
  • DOB and SSN required for consideration

Additional Information

  • Candidates must reside more than 100 miles from the facility to be eligible for this position
  • First-time travelers will be considered if they have relevant home health and/or hospice experience
  • Former permanent staff must wait a full year before returning to this facility as a traveler
  • Orientation is conducted in the field; the office is located in the basement of the hospice house
  • Training and support are available for the dual home health/hospice role
  • Scrub color is flexible — any color scrubs or business casual attire is acceptable
