Key Responsibilities- Provide medical nutrition therapy services and monitor nutritional care for clients and patients across all age groups, including neonatal, pediatric, adolescent, adult, and geriatric populations
- Conduct nutritional assessments, develop individualized care plans, and implement nutrition therapy including diet modification, nutrition support, and oral nutrition supplements
- Deliver education and counseling for disease and nutrition risk management, and evaluate outcomes of medical nutrition therapy interventions
- Support optimization of self-management education and skills for clients and patients with chronic conditions
- Recognize and provide for nutritional needs based on cultural, ethnic, and religious considerations
- Develop and lead nutrition care protocols for assigned areas, serving as a nutrition resource for the broader healthcare team
Required Qualifications- Active Minnesota state Dietitian license required
- Experience providing medical nutrition therapy across primary, specialty, acute, skilled nursing, outpatient, and/or community care settings
- Ability to evaluate treatments and procedures using an evidence-based approach
- Competency in developing and implementing individualized nutrition care plans and provider order recommendations
Preferred Experience / Skills- Experience working with patients across a broad age spectrum, including neonatal and geriatric populations
- Background in chronic disease management and patient self-management education
- Experience leading nutrition care protocol development within a multidisciplinary healthcare team
