Licensing Requirements: Active Oregon State license or proof of Oregon license application and fingerprinting is required with submission. References: One professional reference from a Charge Nurse, Supervisor, or Manager is required with submission. Reference should preferably be from within the last year. Background Review: No felony convictions accepted. Any background hit must include: Traveler explanation statement Waiver/approval documentation (WA) Return Staff Policy Former staff must be separated from the facility/system for at least 1 year before returning as a traveler. Start Facility Requirements : Compliance Documentation: All compliance documents must be completed and submitted by the Wednesday prior to start date. Modules: Pre-start requirements, including modules, skills testing, competencies, and compliance activities, are non-billable. Estimated completion time is 4 8 hours.
