Key Responsibilities- Provide occupational therapy assistant services to patients in a home health setting under the supervision of a licensed Occupational Therapist
- Deliver patient care across an assigned territory including the North Santa Rosa area and surrounding communities
- Meet a daily productivity standard of 6 points per day
- Accurately document patient care and progress using the HCHB electronic medical records system
- Collaborate with the care team to support patient treatment plans and goals
- Flex to assist outside of the primary assigned territory as needed
Required Qualifications- Active COTA (Certified Occupational Therapy Assistant) certification
- Minimum of 1 year of home health experience with a Medicare-certified facility
- Proficiency with HCHB (Homecare Homebase) EMR system required
- Ability to work a Monday–Friday daytime schedule with no on-call or weekend requirements
- Valid driver's license and ability to travel within an assigned geographic territory
Preferred Experience / Skills- Familiarity with Medicare home health guidelines and documentation standards
- Experience managing a caseload across a multi-city or rural service territory
